Preventing Readmissions From Post-Hospital Syndrome

Preventing readmissions is of utmost importance to hospitals. The elderly, however, are particularly vulnerable to what is know as "post-hospital syndrome". Research attributes the stress of a hospital admission to a high rate of readmission for elderly patients.
